Office Policies

 

Insurance

Dr. Kaplowitz is not "in-network" for specific insurance programs.  However, if you have a PPO or POS or indemnity insurance plan you may have "out-of-network" benefits that may cover your visits.  . You will be provided with a statement of services which you may submit to your insurance for reimbursement. 


Payment Options

Payment is expected at the time of service, unless other arrangements have been made;  methods of payment include cash, check , Zelle, Venmo, or credit card.  You are responsible for payment of your medical care regardless of what your insurance company agrees to reimburse.


Cancellation policy 

If you are unable to make a scheduled appointment, it is required that you provide 2 days notice.

Confidentiality

Any information you share is strictly confidential, with certain exceptions required by law.  You hold the privilege of deciding with whom I may disclose information about evaluation and treatment.  If you would like me to share information with other treaters, or anyone else, you will need to fill out a Patient Authorization Form for each individual with whom you would like me to communicate. 

The following situations constitute exceptions to the confidentiality agreement:

  • Suspected abuse or neglect of a minor, elder, or dependent individual

  • A patient is in imminent danger of harming himself or another person

  • A patient communicates a serious threat of physical violence against another person

  • Records are ordered to be released by a judge or court
